What is Form I-693?

Form I-693 is considered a Report of Medical Examination and Vaccination documentation that is required by the U.S. government for immigrants migrating to the US and who want to become permanent residents of the U.S. This Form I-693 ensures that the immigrants don’t have any sickness that could spread to others and that they are in good health. These medical examinations are done to safeguard the health of the American people.

Why should I submit Form I-693?

The USCIS needs to ensure that people migrating to the US don’t have a health condition that might prevent them from being allowed on health-related grounds. Individuals looking for a Green card must submit Form I-693. To complete the form, immigrants should visit a doctor certified by the USCIS or the U.S. Department of Health. You must complete the medical examination and list any health concerns on Form I-693.

How to submit Form I-693?

  • The completed Form I-693 must be in a secured envelope from the civil surgeon.
  • Submit the signed form to USCIS.
  • If the civil surgeon’s paperwork is not in a secured envelope, don’t accept it.
  • USCIS might reject it and send it to you.
  • The civil surgeon will provide you with a copy of the finalized Form I-693 for your proof.
  • Submit your completed Form I-693 to USCIS in a secured envelope. It is valid for two years after the civil surgeon’s signature.
